Age Chart - Baseball Only A matrix chart by month and year that determines the Little League age of baseball players |

Volunteer Application To be completed by any adult volunteer who has regular contact with the players (i.e., managers, coaches, umpires, team parents, board members, snack bar help, etc.) |

Age Chart - Softball Only A matrix chart by month and year that determines the Little League age of softball players |

Combined Teams Application To be used if players from two different leagues will play on the same team. (Example: Two leagues don't have enough players to make one Junior team, the two leagues could apply to combine to make one team.) |

Player Registration Form Used for player sign-ups. Leagues can use this form or opt for their own version. |

Interleague Play Application To be used if two leagues don't have enough teams in a specific division to compete within their own league. This form is typically always used at the Juniors & above level and for leagues without enough teams at the lower divisions. |

|
Player Medical Release Used for authorizing emergency medical treatment. This form needs to be completed by every player and should be done so at the time of registration. Leagues can use this form or opt to use their own version. Managers must carry this with them at all team events (i.e., games, practices) |

Waiver - Regulation IId To be used if a player moves outside your boundaries or your boundaries change and the player wants to remain in your league. |

Proof of Age Little League has tough standards regarding proof of age. This document spells out the required proof/s. This document should be used by the league when registering players. |

Waiver - Regulation IVh To be used if a person had previously resided within the league boundaries for two years while serving that league as a dedicated manager, coach or member of the Local League Board of Directors for two years, and he/she wants his or her sons and/or daughters to be eligible to try out and be selected by teams in that league. |
